Request a Congratulatory Certificate or Certificate of Recognition

Whether you are celebrating a milestone birthday, wedding anniversary, graduation, retirement, volunteer contribution, community achievement, or organizational accomplishment, our office would be happy to honour the occasion with a congratulatory certificate.

If you would like to request a certificate for yourself, a loved one, an organization, or a community member, please complete the form below, and our office will do our best to arrange the timely receipt of a certificate.

To ensure we have sufficient time to prepare your certificate, we kindly ask that requests be submitted at least three weeks in advance whenever possible.
